Keeping you informed about your care is very important to us.

• While you are a patient here, your provider may order tests and/or procedures. The following guidelines will help you get the information you need:

  • Your provider will explain the test or procedure and why you will be getting the test or procedure. It is okay to ask questions if you do not understand.

  • Your provider will explain the benefits and risks to any test or procedure.

  • The purpose of tests and procedures is to help the provider understand your condition and offer the best medical care. You have the right to refuse any test or procedure.

  • The provider or health care team member will verify your name and date of birth prior to performing any test or procedure.

  • Your provider will give you the results of your test or procedure. It is okay to ask what the results mean for your condition or if you need more information.

  • If you have any questions, please discuss them with a member of your treatment team.
